Record of Site Condition Form for Environmental Site Assessment Work
Effective May 1, 2008, a Record of Site Condition form must accompany every Phase 2 environmental site assessment (ESA), remediation report and risk management plan submitted to Alberta Environment. This includes reports conducted on approved facilities, upstream oil and gas sites, petroleum storage tank sites, other commercial and industrial facilities and any other locations. The Record of Site Condition must be completed by a licensed operator or the environmental consultant who conducts the ESA or remediation.
The Record of Site Condition form is designed to track environmental characteristics of a contaminated or potentially contaminated site. It allows site information to be updated as additional Phase 2 ESA and remediation work is completed.
The Record of Site Condition form must be completed, signed and submitted with each of the following documents:
- Phase 2 ESA report for the initial intrusive site investigation to screen for contaminants
- Phase 2 ESA report for delineation of spills and releases to determine the horizontal and vertical extent of the contamination
- Remediation report describing how the contaminated area was remediated
- Confirmatory data report indicating that remediation meets Alberta Tier 1 Soil and Groundwater Remediation Guidelines (Alberta Environment 2009) or Alberta Tier 2 Soil and Groundwater Remediation Guidelines (Alberta Environment 2009).
- Risk management plan or exposure control plan to monitor soil and/or groundwater
2009 Record of Site Condition
On April 6 2009, Alberta Environment released a new version of the Record of Site Condition form. The 2009 version is required for any new information being submitted to the department and for any reports submitted between May 1, 2008 and April 6, 2009 without a Record of Site Condition form. Professional Sign Off
For petroleum storage tank sites and approved facilities, where specified, a professional must sign all reports that are submitted in support of the information provided in the Record of Site Condition form. For upstream oil and gas sites, a professional must sign off on all Phase 2 ESA and remediation work that was completed after January 1, 2008 and submitted with the Record of Site Condition form. Exemptions
This form is not intended for environmental impact assessments (EIA) that may be required as part of environmental impact assessment under Part 2 of the Environmental Protection and Enhancement Act.

The Record of Site Condition form is a public record that is disclosed in accordance with s. 35 of Environmental Protection and Enhancement Act, Disclosure of Information Regulation, and Ministerial Order 23/2004. Reasonable efforts have been made to minimize collection of personal information where possible. Personal information on the form is collected under the authority of section 12(c) and other provisions of Environmental Protection and Enhancement Act and is in compliance with sections 33(a) and 33(c) of the Freedom of Information and Protection of Privacy Act (FOIP). Personal information collected on this form will be used by Alberta Environment for the purposes of administering its programs.
